Pasifika Rise is a Pasifika-led telehealth service. Your trust matters to us, and we take the privacy of your health information seriously. This page explains what we collect, how we use and protect it, and your rights under New Zealand law, including the Health Information Privacy Code 2020 and the Code of Health and Disability Services Consumers' Rights.
Who we are
Pasifika Rise Charitable Trust provides this service. Clinical care and assessment are carried out by Dr Francis Katoa, a Pasifika GP. If you have any questions about your information or this page, contact us at admin@pasifikarise.co.nz.
What we collect
We only collect what we need to assess you safely and provide care. Depending on the service, this can include:
- Your name, date of birth, contact details and, where you provide it, your NHI number.
- Information about your health and wellbeing, including mental health, mood, attention and focus, alcohol or other substance use, and relevant family history.
- Your answers to safety questions, including any thoughts of self-harm, so we can keep you safe.
- Information you choose to tell us in your own words.
You do not have to answer everything, but some questions help us care for you safely. If you leave them out, we may need to follow up before we can offer support.
How we use your information
- To review whether telehealth support is safe and right for you.
- To prepare for and provide your consultation and ongoing care.
- To arrange wraparound support, such as a health coach or social worker, where you agree.
- To keep accurate clinical records, as we are required to do.
How we store and protect it
Your questionnaire and booking details are first received through our website forms, which are handled by our website provider and may be stored on servers located outside New Zealand. From there, your information is transferred into a secure clinical record system used to hold your health record. We take reasonable steps to protect your information from loss, misuse and unauthorised access.
No online service can be guaranteed to be completely secure. Please only share what you are comfortable sending online, and contact us if you would prefer another way to provide sensitive information.
Payments
Payments are processed by Stripe, a secure payment provider. Pasifika Rise never sees or stores your full card details.
How long we keep it
As a health provider, we are required to keep your health information for at least 10 years, in line with the Health (Retention of Health Information) Regulations 1996.
Who we share it with
We do not sell your information or use it for advertising. We share it only:
- With your care team at Pasifika Rise who are involved in your care.
- With your GP or another provider, where you ask us to or agree to it.
- Where the law requires it, or where there is a serious risk to your safety or someone else's.
Your rights
Under the Health Information Privacy Code 2020, you have the right to:
- Ask to see the health information we hold about you.
- Ask us to correct anything you believe is wrong.
- Ask how your information is being used.
