Email a referral
Send to admin@pasifikarise.co.nz with the patient's name, date of birth, NHI, contact number, the reason for referral and any relevant history, results or current medicines.
We work alongside primary care, not instead of it.
Pasifika Rise gives non-urgent, Pacific-led online support for early intervention mental health, ADHD and related care. You refer, a NZ registered doctor reviews, and a clear written plan comes back to your practice.
Email, secure message or phone, with the patient's consent.
A clinician reviews first, then contacts the patient, usually within 24 to 48 hours.
A structured summary and plan returns to you, so care stays joined up.
Pick whatever fits your workflow. All we need is the patient's consent to be contacted and enough context to review safely.
Send to admin@pasifikarise.co.nz with the patient's name, date of birth, NHI, contact number, the reason for referral and any relevant history, results or current medicines.
Call 09 873 5039 to talk through a referral or to set up a standing pathway for your practice or organisation.
Patients can self-refer through the online questionnaire. With their consent we will keep you in the loop, so you stay across their care.
After the consult, your patient's GP receives a structured clinical summary so primary care stays the anchor of their care.
What was discussed, the clinical impression, and any screening scores used, with the tool named.
The plan agreed with the patient, including any medicines started, changed or recommended, with monitoring.
What we will follow up, what we are asking of the GP, and clear safety-netting and red flags.
We are honest about where this sits today, because professional trust matters more than a polished claim.
Summaries are sent to your nominated practice address by a secure channel, formatted to drop straight into the patient record. Tell us your preferred destination when you set up a pathway.
We are onboarding to HealthLink so summaries can be delivered by secure messaging into Medtech, MyPractice and Indici. We will confirm our EDI address with referring practices once it is live.
We contact a patient's GP only with the patient's consent. Information is handled under the Health Information Privacy Code, shared on a need-to-know basis, and kept to what is clinically relevant.
Non-urgent, early intervention mental health support, ADHD assessment and follow-up, prescriptions and reviews, and Pacific patients who want culturally grounded online care from age 5 upward.
Emergencies, acute crisis or risk needing same-day response, and care that needs in-person examination. For urgent help, patients should call 111 or free call or text 1737.
No. We work alongside their usual general practice. With consent, we send the plan back so their GP stays the anchor of ongoing care.
A clinician reviews the referral first, then we aim to make contact within 24 to 48 hours. If online care is appropriate, a secure payment link is sent before the consult.
We screen for risk at review and during the consult. Anyone needing urgent or in-person care is directed to the right service, and we tell you if something needs your attention.
